Facility-Based Respite/Adult Day Care
Facility-based respite care is the provision of relief or rest for a primary caregiver
from the constant, continued supervision and care of a functionally-impaired older person by providing care for the older person in an approved facility-based
environment for a specified period of time.
Adult day care is a program of therapeutic social and health activities and services
provided to elders who have functional impairments. Services are provided in a protective, community-based environment.

How do I Apply?
You may apply for service by calling the Senior Connection Center at 1-800-336-2226. A request for service may be taken directly for the interested person or by referral for others in the community, such as relatives, pastors, social workers, or friends. At this time an initial telephone assessment will be conducted gathering the necessary information needed for eligibility.
